Telehealth In-Home Messaging DeviceLAMP: Low Activities of Daily Living (ADL) Monitoring Program

The Low ADL Monitoring Program (LAMP) serves Veterans who require assistance with activities of daily living (i.e. bathing, dressing, toileting, etc.) in their home. In-home messaging devices and computers are used to provide care coordination. An Occupational Therapist serves as the care coordinator.

Tele Cardiology

The Tele Cardiology program collaborates with Cardiology specialty care to provide Care Coordination Home Telehealth to Veterans with acute heart failure and post MI. An in-home messaging device is used to deliver disease management, patient education, and monitoring support to Veterans, extending primary care into the Veteran’s home.

Tech Care Program

The Tech Care Program collaborates with Primary Care to provide Home Telehealth and coordination of care to Veterans with varying chronic illness such as chronic pulmonary disease, hypertension, mild heart failure, diabetes, chronic pain, malignant disease undergoing radiation or chemotherapy, have had three or more ER visits within a six month period, and offer palliative care for Veterans with advanced illnesses not in hospice care programs. Tech Care also partners with Assisted Living Facilities, and the VFW Retirement Home to assist with coordination of care for frail older adults with complex medical health problems.

Mental Health

The Mental Health Program collaborates with Primary Care and Mental Health Care providers to provide Home Telehealth services to Veterans with Bipolar Disorder, Depression, PTSD, Schizophrenia and Substance Use Disorder. In-home technology is used to provide care and monitor the Veteran's mental health status and other chronic medical conditions, with a goal of restoring the Veteran to their maximum health potential.

Weight Management/Tele-MOVE! Program

The NF/SGVHS Weight Management/Tele-MOVE! Program is embedded in Primary Care, provides care coordination to medically stable Veterans with a BMI >30 who may or may not be in the MOVE program. This program uses in-home messaging devices or interactive voice response systems to deliver patient education and support monitoring to improve the Veteran’s knowledge and self-management of their weight.
