Taylor County VA Outreach Clinic to Close July 19, 2011

The Taylor County VA Outreach Clinic, housed at the Taylor County Health Department, will no longer provide primary care, lab or mental health services to Veterans as of July 19, 2011. In the meantime, all Veterans who have appointments scheduled up to and including July 19 will still be seen at the Taylor County VA Outreach Clinic.
After July 19, Veterans have the option to return to the provider they had prior to coming to the Taylor County VA Outreach Clinic or they may select another clinic location for future primary care services. A VA representative will contact Veterans by phone to discuss options and to arrange appointments with the clinic/provider they select.
If Veterans choose Lake City Medical Center Ambulatory Care Clinic, there is a free shuttle, offered by Big Bend Transit on Tuesdays to the Lake City Medical Center. This shuttle leaves the Annex (across from the courthouse) at 7:30 a.m. and returns after the last patient's appointment. The usual time of return is 3 p.m. Two wheelchair accommodations are available on the shuttle and caretakers are welcome to ride as well. To reserve a space on the shuttle, please call Melody Cox at 850-838-3553 by 12 p.m. Monday or 12 p.m. Friday if Monday falls on a holiday.
