Patient Advocates
The North Florida/South Georgia Veterans Health System (NF/SGVHS) strives to provide the best care possible in the most courteous and efficient manner to our Veteran patients. Patient Advocates are the official representatives of the Director. They serve as liaisons between patients and the medical center, act on the patient's behalf, and help patients understand their rights and responsibilities. They will ensure that the proper individuals address your issues and concerns. Our Patient Advocates are eager to help you with your concern in a timely manner.

Veteran/Family Member Huddle Board
Our health system is always looking for ways to communicate with our Veterans and their caregivers. One way is with ourVeteran/Family Member Huddle Board. This huddle board provides a mechanism for Veterans and their caregiver to bring suggestions or concerns forward in such a manner that it can be used to make improvements to our system. It provides tracking of the progress of suggestions, including their completion with the outcomes. It is also another forum to provide information on events and news within our health system and VA. Suggestions can also be submitted electronically here.
Thanks for a Good Job Card
Your feedback about the care and services you receive provides us an opportunity to make changes and improvements. During your stay or outpatient visit, you may be surveyed by one of our Patient Advocates or receive a "quick card". This is a short patient satisfaction survey asking for feedback about your perception of the care you are receiving. You may answer your quick card anonymously, or you may provide your name and contact information if you would like us to contact you about the resolution of any issues.
Survey of Healthcare Experience of Patients
After your visit, you may receive a confidential questionnaire in the mail asking you about your most recent outpatient or inpatient treatment at our medical center. Please use the survey to let us know of any concerns, complaints, or questions you have about your care, so that we can resolve them. We also appreciate compliments about what we are doing right.

Welcome Home! You served our nation honorably and we are truly grateful for your service. You are eligible to receive many VA benefits, including quality health care. Dedicated VA employees are ready to serve you and make sure you have the smoothest transition possible.
