Self-Referral Direct Scheduling- Clinical Pharmacy

The Department of Veterans Affairs has launched Patient Self-Referral Direct Scheduling (PSDS) for *PACT Clinical Pharmacy Specialist appointments at the North Florida/South Georgia Veterans Health System (NF/SGVHS). This is one of many ways the VA is evolving and adapting to make sure Veterans get the care they need, when they need it.
Currently, Veterans who would benefit from PACT Clinical Pharmacy Specialist services are seen by their primary care provider and consulted for the services. Direct Scheduling for PACT Clinical Pharmacy Specialist appointments makes this process easier and improves the Veteran experience by enhancing Veteran care and satisfaction.
PACT Clinical Pharmacy Specialist services include, but are not limited to: managing diabetes, high blood pressure, high cholesterol, smoking cessation, and chronic obstructive pulmonary disease (COPD).
All Veterans who are enrolled in the NF/SGVHS with a Primary Care doctor are eligible for these services. Veterans may request a PACT Clinical Pharmacy Specialist appointment by calling the Centralized Scheduling Unit at 1-800-324-8387 ext. 106355 or ask your primary care provider.
*PACT (Patient Aligned Care Team) involves each Veteran working together with health care professionals to plan for the whole-person care and life-long health and wellness.
