Safety Committee - Annual Renewal
The purpose of the Subcommittee on Research Safety (SRS) is to ensure the safety of VA personnel conducting VA research. No research project may commence prior to final approval by the Research and Development (R&D) Committee. These procedures apply to all VA research projects regardless of funding source or administration office.
All research studies must have an initial Research Protocol Safety Evaluation completed and submitted at the time of the initial submission to the HRPP Office or the IACUC Office.

For all SRS submissions please provide a COMPLETE electronic copy (including signatures and all attachments), as unsigned MS Word documents.
ALL FORMS MUST BE TYPED, FULLY COMPLETED, AND INCLUDE ALL ATTACHMENTS.

This form will give you a list of all forms/documents needed for your safety submission and will serve as a checklist.
In addition, the second page of this form provides a table where you must list your study PI, Co-PI, Investigators and other staff, and note all SRS-required trainings for each person.
Submit this completed form with the SRS Cover Sheet directly to the Grant Specialists.
Attach all support forms/documents checked on the 1st page of the SRS Cover Sheet.
