Closure Submissions to the VA
**All inquires for the VA study closure process shall be addressed to Sofia Ortiz.
**NOTE: PLEASE CONTACT THE HRPP OFFICE FOR THE MOST CURRENT/UPDATED FORMS**
All forms need to be typed, electronically signed (if required), and submitted via email to the HRPP staff. Your study IRB# should be noted in the subject line of ALL emails to the HRPP office staff. This will ensure proper sorting, filing, and accountability of study materials. Do not scan all forms together all forms are to be scanned separately.
Please remember to submit your closure to the IRB first. ALL studies need to be closed with the IRB. Once you have received your IRB closure approval letter, please submit the IRB closure letter and VA closure forms to the HRPP Office via email to.

Cover Sheet (HRPP Checklist) for ALL other submission packages. This form will give you a list of all forms needed and serves as a checklist.
VA Annual Renewal - marked that the project has terminated
VA Annual Renewal for Exempt/Non-Human Studies - marked that the project has terminated
Abstract - Marked final in word formatNOTE: When ALL forms are received, the study will be submitted to the R&D committee to be officially closed at the VA. You will receive an R&DC closure letter following the meeting.
