The North Florida/South Georgia Veterans Health System’s Health Care for Homeless Veterans (HCHV) Program is designed to provide services to assist veterans who are homeless in becoming self-sufficient and independent. These services include physical and mental health exams, assistance with applications for VA benefits, substance abuse treatment, educational and vocational rehabilitation, assistance with residential care, and case management. HCHV staff work in partnership with various community agencies and provide outreach to community shelters, agencies, and campsites in order to assist veterans who are homeless in obtaining these services.
